Breast augmentation is performed to enhance the size and improve the shape of the breast. Breast augmentation is also effective in treating saggy breasts of minor to moderate severity. Breast augmentation can be achieved by using either breast implants or the patient’s fat. (see Fat Graft)


Worldwide, the most popular method of breast augmentation is breast implants. A breast implant has two main components: an outer shell that is made of silicone, and the content inside the shell that is filled with either silicone gel or saline. The outer surface of the implant is either smooth or textured. One of the benefits of a smooth implant is the ease to insert the implant. On the other hand, the advantages of a textured implant are better tissue-implant interaction and reduced incidence of capsular contracture, a condition that adversely affects the results of breast augmentation especially in terms of appearance and feel. Regarding the content inside the implant, silicone gel provides superior results because of a more natural feel and much lower incidence of rippling and leakage compared to a saline implant. The latest generation of form-stable breast implant is filled with cohesive silicone gel that will maintain its form and shape even when the outer shell of the implant is cut or punctured. This greatly reduces the incidence of implant gel leakage compared to previous generation implants.


Breast implants are available in two different shapes: round or anatomical (teardrop) shape. The round implant is the traditional design and remains very popular especially in the West. A round implant can create a round and well-balanced breast. On the other hand, the shape of an anatomical implant resembles the natural shape of the breast, which is rounder at the bottom and tapered at the top. An anatomical implant is excellent in creating a more natural-looking breast. The anatomical implant is more popular among Asian patients and is suitable for patients with smaller and thinner body frame.


Breast implant is usually placed in either one of the two locations: under the breast or under the muscle. The implant is not located inside the breast itself. The presence of breast implants would not affect milk production and breastfeeding. It is possible to perform mammography on patients who have breast implants.


A breast implant is a medical device that is placed inside the body. Thus, it is very important that the breast implant used is of high quality and is an approved product. In 2006 the U.S. Food and Drug Administration (FDA) officially approved the use of silicone gel breast implants manufactured by approved manufacturers. The breast implants we are using have FDA approval and sourced from licensed local distributors. An official implant ID card will be given to the patient to ensure the authenticity and provide warranty of the implant. Currently our clinic use breast implants from Mentor and Motiva.




1.    Inform the doctor of any pre-existing medical conditions and drug allergy. All medical conditions must be treated and stabilized before surgery.


2.    Stop smoking at least one week before surgery. Smoking is harmful to wound healing and increases the risks of other post-operation complications

3.    Stop taking the following medications and supplements one week before surgery:


a.    Medicine that increases bleeding during surgery such as aspirin, NSAIDs and warfarin. However, you may need to consult your physician who prescribed the medicine before you stop them. 

b.    All supplements that contain vitamin E, ginseng, ginkgo, garlic, fish oil and other ingredients that increase bleeding during surgery. Other supplements, traditional medicine and herbs in which ingredients are unknown have to be stopped as well.

4.    On the day of surgery, wear simple and comfortable clothing and do not wear any makeup. Do not wear any jewelry and metal objects on the face and body.





Duration: 1-2 hours

Anaesthesia: General Anaesthesia

Hospitalization: Half a day (daycare)

Recovery*: Back to work in 3-7 days, light exercise after 1 week, heavy exercise after 4 weeks. * The actual speed of recovery may vary from person to person.

Surgery Technique: There are 3 routes through which the implant is inserted. The selection depends on the indication of breast augmentation, the patient’s anatomy, and the patient’s preference.

1.    Infra-mammary route: The incision is located on the lower boundary of the breast, the place where the breast and the chest meet. Worldwide, this is the most popular option used for breast augmentation. The technique offers excellent control and safety during pocket dissection and implant placement. The scar is well hidden under the breast and usually heals very well and will not be obvious after full recovery.

2.    Periareolar route: The incision is made around the lower half of the nipple-areola circle, at the junction between the darker areola and the lighter surrounding skin. Scar is usually not obvious. However, this technique is not suitable for patients with small areola because the incision will be too small for the implant to pass through.

3.    Transaxillary route: The incision is made in the armpit. The use of endoscopic camera and dissection enables accurate pocket dissection and implant placement. The scar is located in the armpit instead of on the breast.


Post-operative Care**

1.    What to expect: Swelling usually peaks on the second to third day after surgery and will gradually subside thereafter.

2.    Tight sensation in the chest may be present for one to 2 weeks especially if large implants are used. A supportive bra is used to support the breasts for the first one month.


3.    General care:

a.    Avoid smoking for at least one month. Smoking increases the risk of wound complications.

b.    Sleep on the back. Avoid sleeping on one side to prevent pressure on the breasts.

c.     Good rest and adequate sleep are helpful for a speedy recovery.

d.    Be relaxed and calm. Contact the clinic if there are any queries.

4.    Medicine: Finish the oral antibiotics as prescribed. Take the painkiller as prescribed when necessary.

5.    Wound care: After the dressing is removed, clean the wound with a clean cotton tip soaked with sterile water/saline. Apply antibiotic ointment.

6.    Physical activity: Avoid heavy physical activity and exercise for at least one month.

7.    Follow-up: Come back one week after surgery for review.

8.    Emergency: If there is heavy bleeding, rapid increase in swelling or severe pain, immediately contact the clinic/doctor for advice.
